Guidelines for New Students to USD 413
Students entering Chanute Public Schools for the first time (regardless
of grade level) must meet the following admission requirements:
- School Age Requirements: Children five years old on or before
August 31 are eligible for kindergarten enrollment. Those six years
old on or before August 31 may enroll in first grade.
- Proof of Identity: All students enrolling in the district for
the first time are required to provide proof of identity. Students
enrolling in kindergarten or first grade will need to provide a
certified copy of their birth certificate or other acceptable documentation.
Proof of identity may include a copy of a court order placing a
child in the custody of SRS, a certificate of transcript, or a
baptismal certificate. A copy of the child’s social security
card and a certified birth certificate are preferred. Students
enrolling in grades 2 – 12 need to provide a certified transcript
or similar student records from their previous school as proof
of identity.
- Immunizations: Students enrolling in Chanute Public Schools must
comply with immunization laws and school board policy. Proof of
immunization for DPT (diphtheria, tetanus and pertussis), poliomyelitis
and MMR (mumps, measles and rubella) is required. Booster shots
are also required by State law. Parents should contact their physician
or the county health department for immunization forms and procedures.
Questions may be referred to the school nurse, 432-2530, or the
building principal.
- Student Health Assesments: Every pupil, up to the age of nine,
shall present the results of a health assessment prior to entering
kindergarten or before enrolling in the district. All students
engaged in activities covered by appropriate Kansas State High
School Activities Association rules shall provide the building
principal with proof of a physical examination before participating
in these activities.
- Out-of-District Students: New students who are not residents
of U.S.D. #413 may be eligible to enroll upon application to the
superintendent. Students may be admitted only to the extent that
staff, facilities, equipment, and supplies are available. Any student
who has been suspended or expelled from another district will not
be admitted to the district unless approved by the board of education.
Out-of-district students may be enrolled in U.S.D. # 413 schools
for a probationary period before final approval is given. However,
the school district reserves the right to revoke acceptance at
any time due to unacceptable attendance or behavior records.
